Looking under my hood it looks like there is a mount for a jack set up on the driver side. Am I correct? Does anyone have a pic of this original set up? Thx

No pic but you are correct. The OEM jack mounts on the inner fender, near the front. There's a bracket that flips over it and screws down with a wing nut. Here's what it looks like http://www.fvstore.com/1967-1968-1969-1 ... 23596.html Check out that price!

I picked up a bottle jack from a 96 F150 at the pic a part yard for $15. It fits perfect
